To register for PRISM or WinPRISM training, or to find out about availability of any classes, please contact:

When registering, please provide the following information for each training participant:
     *  Name (please indicate salutation Ms or Mr)
     *  Position or Title in store
     *  Store name
     *  City and State/Province
     *  Email address
     *  Whether the participant is flying or driving to Lincoln
     *  Whether the participant requests a smoking room.  All rooms are reserved as non-smoking unless otherwise
        requested.
     *  If registering more than one participant, let us know if anyone plans to share a room at the hotel.  All rooms are
        reserved as singles unless otherwise requested.

IMPORTANT:
If you must cancel registration after enrolling, please contact Michelle Sackett. Failure to cancel a registration less than two weeks before that session may result in your bookstore being charged for the space in the class.
